1. I/We hereby declare that the particulars provided above and the documents provided along with this form are true, correct, and complete to the best of our knowledge and belief and have been legally obtained from the respective issuing authorities.

2. I/We hereby declare that we are not required to obtain registration under the GST law as we do not meet the turnover threshold limit for registration as per the applicable GST law. In the event of registration taken by us under GST law, providing such registration information to Wellnessta immediately shall be our responsibility. In the future due to any reason if the vendor(Service provider) is liable for registration then all TCS liability / Penalty etc will be of the vendor (Service Provider).

 3. In case of any change in any of the aforementioned particulars, we undertake to notify Wellnessta immediately in writing. We shall be personally responsible and liable if any of the information mentioned above is found to be incorrect and we shall indemnify Wellnessta Private Limited for any loss incurred to that extent.
